Elementary Calculus: An Approach Using Infinitesimals by H. Jerome Keisler
He's made it available online at http://www.math.wisc.edu/~keisler/calc.html
I believe it's the only infinitesimal calculus textbook that is as comprehensive and intended as a complete textbook.
